Adult's Mental Health Social Worker - Hybrid - London

A Local Authority in London is hiring for a full time Adults' Mental Health Social Worker to adhere to local adults in need that are in use of the support from adult's social care as part of the mental health teams.
Key Duties include:
Initial Mental Health Assessments and Support Plans
Understand DTOC (Delayed Transfer of Care)
General Case Management and completing needs assessments

Qualified social worker with 2 year + adult mental health experience (expected)
Registered with SWE (expected)
Professional qualification in Social Work (expected)
Understand hospital discharge and DTOC (delay transfer of care)
